I finished the Five Wishes document AND realized that my next door neighbors (other side than Hunter) were probably both there and ok with witnessing. They run an insurance office out of their unit and actually live in Bellevue. I knocked on their door and they were both right there in the front room and happy to do it. So DONE!!

I actually put it in a public folder because why not?

The document is so that my brother will know exactly what my wishes are for the end of my life should I not be able to express them. In other words - pull the plug, please!!!

That looks like a great document, I might have to buy one. I downloaded one last year off a legal site and it was so convoluted I tossed it out after reading two pages. A living will is something on my list of things to take care of this year. Thanks for sharing that!

Interesting... I am the very opposite. If I don't know and Google won't tell me, I stay dumb. I rarely ask.

Just glanced over the Five Wishes, curious to see what this is about. On the first side though I was wondering if your birthdate is correct? 1904?

I'm glad that this was helpful. But look at my mistake: on the first side... Of course this should be "on the first page". It dawned on me when I went to bed and tried to fall asleep. Thinking's not always easy.

I wish everyone would fill one of these (or something similar) out. I can't tell you how many times, as an ICU nurse, I saw people be subjected to treatments they probably would have never wanted, for lack of some clear guideline for their families.
